Share the Care Home Care Program
Share the Care Home Care Program has developed a unique service for the residents of senior living apartments. Traditionally, if the resident of a senior apartment were to need minimal assistance with certain activities, a private duty aide would have to be hired. Private duty can be expensive and often requires time minimums, set with no regard for the activities accomplished.
If service was unavailable or unaffordable many residents would be forced to move into long-term care facilities or even nursing homes.
What is Share the Care?
Share the Care is assistance with activities of daily living with rates far below those of private duty home care services. The savings are made possible by the skillful scheduling of services for multiple residents in block times.
What services are available?
Share the Care was designed to meet the “individual” needs of each resident the same as private duty home care services with one exception-the residents share the cost. The client only chooses and pays for the services needed.
Example of Services:
Showers: Assistance is provided with undressing, getting in and out of the shower/ tub, bathing, toweling dry, and/or dressing.
Med Pass: Field staff employees will visit at specified times to remind and assist in taking medication.
AM & PM Care: Morning visits consist of preparing a continental breakfast, assisting with dressing, hygiene activities and daily activity planning. Evening visit includes a review of the day’s activities, preparing a snack, getting client undressed, assist with hygiene activities and tuck into bed.
Incontinence Checks: Assistance the client with reminders or with changing or/and utilizing the restroom.
Light Meal Prep: As with continental breakfast, sandwich and soup, light evening snacks.
Bedding Change: Field staff employees will change the bed linens.
Assistance with Feeding: Field staff employees will assist with assuring that the resident is eating and drinking fluids.
Laundry Service: Assistance is provided with laundry including washing, drying, folding and putting away.
Light Housekeeping: Includes; vacuuming, dusting, changing of bedding (if requested), clean toilets, showers and tubs, mop floors, clean mirrors, and wipe up spills. In general, leave the home clean and tidy. This does not include; moving furniture, climbing ladders, scrubbing floors on hands and knees, polishing or removing books or objects that can be broken.
Assistance with Prescriptions: Eye drops, topical creams and ointments, patches etc.
Monitoring Checks: Blood sugar monitoring, blood pressure monitoring, etc.
Bed Bath: Assistance is provided with undressing, bed bathing, toweling dry, and/or dressing.
Private Transportation: To physician and other important appointments.
Hourly Private Duty: For hospital discharged residents and special needs clients
Share the Care Staff
Share the Care services are provided by the same field staff as if you hired them privately. Each employee is trained in all aspects of private duty home care and is dedicated to maintaining and improving the ability of their residents to function independently.
